What is intermediate care?

If you're well enough to leave hospital but you still need help to live independently, you might be eligible for free, short-term care and support. This is intermediate care, which is sometimes called a ‘reablement’ service.

Intermediate care is designed to help you recover and become independent in your own home again following a hospital stay. You can receive it for up to six weeks if you live in England. It could include:

  • nursing care to help you manage your condition  
  • personal care, such as washing and dressing

You might get intermediate care at home, in a care home, or in a community hospital.
