Assessment process procedure for assessing volunteers (DOC) This document goes through the process of when and how you will be asked to do an assessment, and how to make an assessment of older person who has been referred to the service. This document is key to your role so please take the time to read through it carefully and speak to your Independent Age contact with any questions you may have. Service User boundaries agreement (DOC) This documents outlines what a service user can expect from our volunteers, and also what they are unable to do for them. Again this is a really important document and one of which needs to be agreed to and signed by the older person who is receiving the service. Stage 2 - Assessment form (XLS) This Stage 2 assessment form gathers information about the older person’s current situation around social activities and loneliness/ isolation. It also looks at risk related to any health issues and their accommodation. Stage 3 - Call template (DOC) This document lists all the questions you should ask when booking in your stage 3 assessment visit with the older person you are due to visit. Once this form is completed and a suitable time booked in please send a meeting confirmation letter. Stage 3 - Meeting confirmation letter (DOC) This is a template letter for you to use when confirming a meeting with an older person to make a stage 3 assessment. Stage 3 - Assessment form with guidelines (XLS) This is the stage 3 assessment form. It includes a script to encourage service user participation, questions to get to know the individual further for example their hobbies and daily routine. It also looks at the wellbeing and loneliness scales, and has the consent form which needs to be signed. Stage 3 - Assessment form with answers (XLS) This is an example of what a completed stage 3 assessment form should look like after a visit with a service user/older person. Psychological Scales Answer Sheet (DOC) This document lists a number of psychological scale questions and suggested answers for you to guide service users through to assess how they are currently feeling.
